「解決問題」在本書採用狹隘的定義：「如果某個系統之內發生了錯誤而導致系統的產出變差了，而我們把這個錯誤移除了而讓系統的產出恢復正常。」這種方法論，在醫學的領域，叫做診斷；在應用軟體開發的領域，叫做除錯；在水電工的領域，叫做抓漏。
而「創新」在本書的定義則是指：「對系統做出干預，以提高某個系統產出的天花板」。所有的組織都需要創新，不創新的組織註定要死氣沉沉。但是，要怎樣才能創新呢？創新有什麼方法論可以參考嗎？
